Welcome to Benefits Open Enrollment 2022

CVA has been working diligently with multiple brokers to identify the best benefits for our employees at the most affordable rates. 

For plan year 7/1/22 – 6/30/23 CVA will be working with Consociate Health to negotiate reduced cost for surgeries and other procedures with Health Link being our our network for health insurance. All 3 health link plan options are the same as last year and have the same deductibles, co insurance, copays, etc. If you have an upcoming procedure, you can report this information to Curtis Mason, IT/HRIS who will notify Consociate Health and they can reach out to the hospital or other provider to determine if there is a savings opportunity for employees. 

Our Pharmacy provider for this plan year will be EmpiRX. Below you can find the complete formulary list of prescriptions along with the preventative drug list. 

Dental & Vision will again be provided by Delta. Rates and plan coverage have stayed the same. 

Life Insurance coverage continues to be through Guardian, with Full-Time and Part-time employees working 20+ hrs per week receiving the CVA employer paid 15,000 coverage for life/AD&D, with the option to purchase additional voluntary coverage. 

CVA will continue to contribute $100.00 a month to HSA accounts. 

CVA will continue to contribute $40.00 a month to FSA accounts. Paylocity will remain the account vendor for employee FSA accounts and no action is needed by employees other than to update their contributions during enrollment.

Enrollment will be completed online via Employee Navigator.  Please access Employee Navigator www.employeenavigator.com and click on the Login page and register as a new user with company identifier as “Chariton“.

Discussion meetings for Full-Time Employees will be:

Mon June 13 8:30am and 3:30pm

Wed June 15 8:30am and 3:30pm

Fri June 17 8:30am and 3:30pm

Mon June 20 8:30am and 3:30pm

Tues June 21 8:30am and 3:30pm

Discussion meetings for Part-Time Employees (20+ hours per week) will be:

Mon June 27 8:30am and 3:30pm

Tues June 28 8:30am and 3:30pm

Thurs June 30 8:30am and 3:30pm

You must go online to the mymitc training calendar and enroll in a time slot to reserve your spot. 

HSA Accounts with High Deductible Health Plan

If you enroll in a High Deductible HSA Health Plan, you will need to setup an HSA account at your local bank.  

There are several local banks in Kirksville that offer an HSA account including:

You can choose which bank you prefer to open your account. You will need to provide Payroll an HSA direct deposit form to have your contributions deposited into your new account.
